1) Create a document that will be used in a mid-sized company, by employees, to submit their travel expenses. The document must be able handle the following information:
a) Employee Name, Number, Department
b) Reason for trip
c) Location
d) List of transportation expenses with their prices
e) List of living expenses with their prices
f) Sub-Total
g) Total
h) Signature of Employee
i) Signature of Authorization
f) Check boxes for currency
The document should also have the company logo in the top left hand corner. The document must fit on one page only.
